Rail Act is all about the balance. In each level rotate the rail so that the ball falls in the area designated by the purple or yellow button. Holding the ball in button's place for a moment will advance you to next level. Lives are indicated with a number in the top right corner of the screen. Lose all your lives and you will have to go back to first stage, there is no save, you have to finish the game with the balls you have.
